DIRECTIONS:

1. Just provide the first and last name (please MAKE SURE TO CAPITALIZE the first letter of the first and last name, example: John Smith --- not, JOHN SMITH).

3. Enter mobile phone number if you want the customer to receive a SMS text message to leave a review.
4. Click the submit button. The customer will now receive an email inviting them to provide us with a Google, Facebook, and/or website review.

BEST PRACTICES

When a customer's order has been fulfilled, thank them for their business and let them know they will be receiving an email shortly and we would love if they would provide a quick rating and review of our services.

The SOONER you complete the form following the customers order the better the result of them providing a review.
